If you want a watch-only experience, the app does not need the iPhone once the app is loaded on the watch. If you see value in keeping track of the history of your counters, you will want the optional in app purchase to show the counters in a table on your iPhone. If you need only one counter, you get it right away. If you want multiple counters and want to change the names of your counters at any time, then you need the optional in app purchase to allow five counters.
